Enrolling in the Medicare Advantage Plan
Members who are not currently enrolled in the UCC Medicare Advantage Plan should complete the applicable enrollment/change form and return it to PBUCC with a copy of the Medicare card. Applications must be received during the applicable Open Enrollment period.
Members who want to cancel current coverage should complete the applicable enrollment/change form and return it to PBUCC during the applicable Open Enrollment period.
