Participants in the UCC LIDI Plan may purchase additional life insurance coverage for themselves and their eligible dependents.

Additional Life Insurance Coverage for Yourself

If you are covered under the basic LIDI Plan, you may request additional life insurance coverage. This special opportunity enables you to replace basic Plan coverage (which has a benefit payout that decreases as you get older) with additional coverage to increase your life insurance benefit.
Eligibility

You may request additional life insurance coverage of $10,000 within your initial 90 days of eligibility without a review of health. If you request any amount over $10,000, or apply for any increased benefit outside your initial 90 days of eligibility, you must submit a completed Statement of Health along with the enrollment form and Metlife Change form.

Benefit Coverage and Cost

The minimum additional life insurance coverage you may request is $10,000. The maximum coverage is the lesser of three (3) times your annual salary basis, or $100,000, available in increments of $10,000. In determining the maximum, your basic life insurance coverage provided under this Plan is not counted. For cost information click here.

Spouse/Partner Life Insurance Benefits

If you are covered under the basic LIDI Plan, you may request additional life insurance coverage for your spouse/partner. Spouses/partners of active members covered under the basic Plan are covered by a death benefit of $5,000. You may purchase an additional $10,000 or $25,000 of coverage for your spouse/partner. Dependent Child(ren) Life Insurance Benefits

An employee participating in the basic LIDI Plan may purchase $5,000 or $10,000 coverage for dependent child(ren). Click here for the enrollment form and MetLife Change form.

Eligibility

If you are within your first 90 days of eligibility for the Plan, you may apply for $10,000 of additional coverage for your spouse/partner without a review of health unless your spouse/partner has been hospitalized in the past 90 days. If you are applying for $25,000 of coverage for your spouse/partner, the completed Statement of Health form will be required along with the enrollment form and MetLife Change form.

You may apply for additional coverage for your dependent child(ren) within your initial 90 days of eligibility without a review of health unless your dependent(s) has been hospitalized in the past 90 days. If a hospitalization has taken place, a completed Statement of Health will be required along with the enrollment form and MetLife Change form.

You may also enroll within 90 days of your marriage, birth, adoption, or legal placement of child in your care without a health review, up to $10,000. Any amount requested over $10,000 (regardless of when you apply) will require a completed Statement of Health along with the enrollment form and MetLife Change form.

You may apply for coverage for your domestic partner within 90 days of the six-month period following the commencement of your domestic partnership or civil union (whichever is the earliest), up to $10,000, without a health review. Any amount over $10,000, regardless of when you apply, will require a completed Statement of Health along with the enrollment form and MetLife Change form.

Benefit Coverage and Cost

The spouse/domestic partner of an employee participating in the LIDI Plan is covered by a benefit of $5,000 (with no coverage for dependent child(ren) at no additional cost beyond the standard monthly premium. You may add significantly to this coverage by purchasing either an additional $10,000 or $25,000 of coverage for your spouse/partner and either $5,000 or $10,000 of additional coverage for your dependent(s). Discontinuation of Coverage After Employment Ends

Additional/optional life insurance coverage will end when you cease to be a member of the Plan, when premiums are no longer paid, if the contract with MetLife is cancelled, or this Plan is terminated. If your employment ends, you will no longer be eligible to continue this coverage through the Pension Boards. You do, however, have the option of converting it to an individual policy with MetLife.

The LIDI Plan's Basic Life Insurance coverage is designed to provide UCC clergy and lay employees and their families with financial peace of mind in the event of death.

Normal Age Retirees who are age 65 or older and have participated in the LIDI Plan for five (5) or more consecutive years receive $10,000 of life insurance coverage at no cost.

Early or Normal Age Retirees who participated in the Plan for less than five (5) consecutive years will receive a benefit rounded to the nearest $100, but not more than $2,000.

Additional Voluntary Life Insurance

The minimum amount of optional additional life insurance coverage you may request for yourself is $10,000. The maximum coverage is the lesser of three (3) times your annual salary basis, not to exceed $100,000. Click Here for more information.

Disability Income Benefits

The LIDI Plan also provides active employees with life insurance and income replacement while they are on short-term disability (STD) or long-term disability (LTD).

Eligibility

Full-time* authorized ministers and lay workers employed by a UCC church or UCC-related entity are eligible to apply for LIDI Plan coverage.

An authorized minister includes ordained, commisioned and licensed ministers:

  • with ministerial standing in an Association of the UCC (or a Conference acting as an Association) and employed by a church or other religious or charitable organization, whether or not affiliated with the UCC; or,
  • with ministerial standing but self-employed in a field which, in the judgment of the Pension Boards, is related to the mission of the UCC.

A lay employee is someone who is a non-ordained, non-licensed, or non-commissioned employee of a UCC church or related organization.

*An employee who works 20 or more hours per week is considered to be employed full time. Coverage in this Plan is on an individual basis. Only the employee for whom the premium is paid is covered.

Cost

The annual premium for life and disability coverage is 1.5% of salary basis (salary plus housing allowance).

How to Enroll

You can enter the Plan without providing evidence of insurability if you submit your application within 90 days of your first eligible full-time (20 hours or more per week) UCC employment. If you do not apply for coverage under the Plan within 90 days of your first eligibility, you must submit a Statement of Health form along with your application. An independent underwriter will review your Statement of Health and determine whether or not you can enroll in the Plan. Coverage will become effective on the first day of the following month after the acceptance of your Statement of Health.

Optional Additional Life Insurance

If you are covered under the basic LIDI Plan, you may request additional life insurance coverage. This special opportunity enables you to replace basic Plan coverage that will decrease as you get older, or to increase your life insurance.

Eligibility

If you request additional coverage within 90 days of joining the Plan, you will not be subject to a review of health. If you do not request additional coverage within 90 days of joining the Plan, you must wait until the annual end-of-year open enrollment period to apply.

The minimum additional life insurance coverage you may request is $10,000. The maximum coverage is the lesser of three (3) times your annual salary basis, or $100,000, available in increments of $10,000. Dependent Death Benefits

If you are covered under the basic LIDI Plan, you may also request additional life insurance coverage for your spouse/partner and optional life insurance coverage for your dependent child(ren).

Eligibility

If you do not have a spouse/partner or dependent when you are first enrolled in the Plan, you must request life insurance coverage within 90 days of your marriage or within 90 days of the birth, adoption or legal placement of a child in your care. You may apply for coverage for your domestic partner without providing evidence of good health within 90 days of the six-month period following the commencement of your domestic partnership or civil union (whichever is the earliest).

Disability Plan Facts

SP disability

Short-Term Disability (STD)

Eligibility:

Members who are covered by the United Church of Christ Life Insurance and Disability Income Benefits Plan (LIDI), after being out of work for 30 consecutive days, if approved by MetLife.

Member's Responsibility:

Contact MetLife, at 1.800.300.4296 to apply for STD, if the member's physician has determined that disability will exceed 30 days. MetLife will inform the member and the Pension Boards of approval or denial of the STD claim.

MetLife's Responsibility:

To send a weekly check to the member that is 60% of the salary basis (cash plus housing allowance). The minimum weekly benefit is $20; maximum weekly benefit is $1,384.62. Maximum duration of STD is 22 weeks.

NOTE: For pregnancy, short-term disability benefits are paid without regard to the 30-day waiting period. Unless complications warrant an extension, six weeks of short-term disability payments are provided for a normal delivery and eight weeks for a Caesarean section.

Employer's Responsibility:

For the duration of STD (maximum 22 weeks) the employer continues to pay Annuity Plan contributions, and premiums for the LIDI Benefit Plan and the UCC Health/Dental Benefit Plans. Salary should not be paid while the member is on STD. If salary is paid, it reduces the amount of STD payable dollar-for-dollar.

Taxes:

STD benefits are taxable income. The member will receive a Form W-2 from the MetLife after year-end. Benefits paid to clergy with standing are eligible for the housing allowance exclusion. Clergy will also be sent tax resources to assist them in claiming the housing allowance exclusion.

SP longtermdisabilityLong-Term Disability (LTD)

Eligibility:

Members who are covered by the United Church of Christ Life Insurance and Disability Income Benefits Plan (LIDI), after being on short-term disability (STD) benefits for 22 weeks, if approved by MetLife. If the member remains disabled, the LTD benefits generally continue to age 65.

Member's Responsibility:

To provide medical information requested by MetLife; apply for Social Security Disability benefits and provide MetLife with proof of application; to inform MetLife of approval or denial of Social Security Disability benefit claim.

MetLife's Responsibility:

To send a monthly check, based on 66.67% of the salary basis (cash plus housing allowance.) The minimum monthly benefit is $100; maximum monthly benefit is $6,000. Benefits are reduced dollar-for-dollar by Social Security Disability Benefits awarded to the member. MetLife pays Annuity Plan contributions at 7% of salary basis for members of the Annuity Plan for the United Church of Christ, and provides premium free health, dental and LIDI coverage.

Employer’s Responsibilities:

None. Once short-term disability ends, the employer is no longer responsible for making any contributions or paying any premiums or salary.

Taxes:
