The UCC Medical Benefits Plan includes optional vision coverage among its Plan offerings. Vision benefits are available through VSP, a nationwide network of vision care providers. For a summary of the benefits available, click here. 

• Open Enrollment
  An annual open enrollment period is offered from March 1 to March 31 every calendar year, with coverage effective at 12:01 am on April 1, following receipt of a completed enrollment application along with a check for the annual premium.

• Enrollment After the Annual Open Enrollment Period
  The Vision Benefits Plan is offered on a Plan Year basis from April 1 of the current year to March 31 of the following year. Eligible participants (see "Eligibility," below) may enroll in the Plan after the annual open enrollment period; however, coverage will begin on the date that the completed enrollment application and check for the annual premium are received. Unlike the open enrollment period, benefits are not retroactive to April 1.

• Existing Vision Plan Participants
  Renewal invoices are sent out to existing Plan participants prior to the start of the new Plan Year. If you do not re-enroll for coverage, then decide to rejoin the Plan at a future date, you will be billed retroactively for the lapsed coverage period to ensure that you (and your dependents, if any) receive uninterrupted benefits according to the terms of the UCC Vision Benefits Plan.

• Eligibility

  Vision coverage may be purchased by:

1. Employers for their employees;

2. Individual clergy and lay employees serving local churches; and

3. Retired clergy; and

4. Retired lay employees.

Note: Coverage is offered to the individuals listed above on a stand-alone basis, regardless of participation in the UCC (Non-Medicare) Health Plan or UCC Medicare Supplement Plan.

Participants must apply for individual coverage in order to apply for dependent coverage.
